Welcome to your practice

As a patient-oriented practice, committed to recognizing the individual needs of each person, we have answered a few of the most common questions we are asked. On this page, you will also find some forms we may ask you to fill out.

Our Services

A comprehensive list of the services we offer can be found in the dropdown menu at the top of the page under ‘Services’. Each page provides some FAQ’s and general information. For further information or questions, a discussion with our receptionists or a consult with one of our dentists is the best course of action.

Payments

We accept a range of payment methods including EFTPOS (excluding AMEX), cash, cheques and direct deposits. On a case by case basis, we are able to offer payment plans. This requires a consultation with the dentist before we can issue approval. More information is available upon request.

Health Insurance

We accept all health funds at Unique Dental Care. We have two HICAPS machines available at our reception area to make the process as convenient for you as we can. We require your physical card at the time of your appointment. Some health funds have moved to provide online cards which can be used to ‘tap and go’ - our machines are also equipped to accept this method. Should you leave your card at home on the day of your appointment, we are happy to email you your invoice so you can claim directly from your heath fund provider.

New Patient Forms

Here you will find our new patient forms. We have electronic and hard copies available in our surgery and request that each of our new patients complete this before their appointment. For a streamlined experience, we invite you to fill this form out online. 

Medical History Update

Under the Australian Dental Association (ADA)’s regulations, we are required to update your medical history once every two years. While we understand that often, nothing has changed for your personal medical history, however it is still imperative to our ability to provide you with the best care possible. We have electronic and hard copies available at the reception desk, however to shorten your waiting time, we invite you to download and complete the form and bring it with you to your appointment or email it to reception@uniquedentalcare.com.au

Dr Rawan

Dr Rawan will be back in our practice for a few days this year. She currently has appointments available on the 29-30th of January. Please call reception for more information.

Dr Abdullah Hassan has officially joined our practice as our newest associate dentist. Appointments are now available.
